Service Components

Family Advocate Visits will provide families with peace of mind by offering thorough assessments of their loved one’s living conditions in assisted living facilities. Our trained advocates will conduct regular visits to assess cleanliness, safety, and the well-being of the senior, documenting everything and offering insights that can help improve their care and quality of life.

Routine Visits:
  • Weekly or bi-weekly visits by a trained family advocate to check on the senior and their living environment.
  • Visits will include conversations with the senior, a review of their appearance, and a detailed observation of their room or personal living space.
Comprehensive Facility Assessment:
  • Evaluation of the overall cleanliness of the facility, including public areas such as dining rooms and activity spaces.
  • Observation of staff interactions with residents, ensuring respectful and attentive care.
Personal Care Review:
  • Check on the senior’s personal hygiene, including if they appear clean, well-groomed, and if they’re wearing fresh clothes.
  • Review of bedding, clothing, and room tidiness to ensure basic care standards are being met.
Safety and Comfort Checks:
  • Ensure the room is free from hazards, such as spills or clutter that could cause falls.
  • Confirm the senior’s room is up to standard in terms of temperature, lighting, and accessibility.
Report to Family:
  • After each visit, the advocate will prepare a detailed report, including observations of the facility’s cleanliness, the state of the family member, and any concerns that need attention.
  • Photographs of the living space (with the facility’s consent) can be included in the report to provide visual updates.
Recommendations:
  • Advocates can also provide guidance on how to address any specific needs or improvements that are required.
